KYC Documentation

Identity Proof

This can be any government-issued photo ID such as Aadhaar Card, Passport, Driving License, Voter ID Card, or PAN Card. Please ensure that the Full Name (with exact spelling) and Date of Birth are identical in all KYC documents submitted, medical documents including prescription, and those provided at the time of Registration with the Hospital. Any discrepancy may result in the denial of approval by the Insurance/TPA, and payment for the treatment may have to be borne out of pocket by the patient.

Please note that the following documents are mandatory to be submitted by the beneficiary for availing cashless treatment at Sir Ganga Ram Hospital

· Aadhar Card and PAN card of both the patient ( beneficiary) and Policy Holder.

· Passport size photograph of the patient ( beneficiary) and Policy Holder.

Address Proof

This can be any government-issued document that shows your current address, such as Aadhaar Card, Passport, Driving License, Voter ID Card, or utility bills.
