AIM
The Paediatric
department at Centre for Child Health, Sir Ganga Ram Hospital is committed to
provide best of comprehensive medical care to children below 16 years. The
department of Paediatrics strives to be the leader, in the field of childcare
through development of its paediatric super-specialties, research, community
services and quality professional education.

The department of
Paediatrics is one of the three departments under Centre for Child Health. It is
the biggest amongst the three departments. A chairperson heads the department.
The Director, Centre of Child Health provides guidance and suggestions to the
department of Paediatrics wherever needed.

MEDICINE
Adolescent clinic run
by team of professionals works with adolescents in the age group of 10-18 years
with psychological problems and psychiatric disorders such as depression,
anxiety problems, low self esteem, oppositional defiant behavior, drugs use,
psychosis etc. Our DNB students attend the clinics to get an understanding of
this relatively new specialty. As extension of our services outreach lectures
and sessions at school levels for teachers and students are conducted from time
to time.

PSYCHOLOGY
Child & Adolescent
Psychiatric services are delivered through outpatient and inpatient services.
Outpatient services include Child Guidance Clinic and Adolescent Clinic. Child
Guidance clinic has a team of professionals comprising of child psychiatrist,
clinical psychologist and special educator. Team of professionals comprising of
adolescent psychiatrist, clinical psychologists and adolescent health
specialists runs adolescent clinic.
Child guidance clinic
specializes in working with children with emotional, behavioral and neuro-developmental
disorders such as learning disability, attention deficit hyperactivity disorder
(ADHD), separation anxiety disorder, autistic spectrum disorders, conduct
disorders etc. Self-help groups and awareness programs for parents and
caretakers of children with autism, mental retardation, ADHD and learning
disability are also organized at regular intervals to help and support

A paediatric
psychiatrist runs these services. The facilities are available to treat and
guide all kind of Paediatric psychiatric illnesses ranging from anxiety,

DEVELOPMENTAL
PAEDIATRICS
In child development
clinic comprehensive evaluation & management of these children is done by a
multidiscplinary team under one roof . The team includes developmental
Paediatrician, Paediatric neurologist, Paediatric psychiatrist, child
psychologist, speech therapist, special educator & occupational therapist.
We have formed a
parent support group to provide emotional & psychiological support to the
parents of developmental disabilities. We have published & presented number
of papers various prestigious national & International conferences including
AAP (American Academy of Paediatrics)

EMERGENCY
MEDICINE
Round the clock on all
days (24x7) services are provided for Paediatric emergencies by department of
Paediatrics. The senior resident sees all paediatric patients on duty. Those
having non-urgent problem are advised home treatment and follow in outpatient
department. Those who need hospitalization are admitted in Paediatric ward,
daycare or paediatric intensive care unit according to clinical condition and
care needed for the child. A consultant specially trained in emergency
paediatrics supervises these services. In case the child is critically sick and
bed is not available in the department of Paediatrics, child is transferred to
other health care facilities by medical ambulance. A doctor accompanies the
patient.
Paediatric procedural
sedation and analgesia services are also provided by the Paediatric emergency
medicine professional. These include facilities for minimal to deep sedation.

ENDOCRINOLOGY
A trained and
experienced paediatric endocrinologist runs these services. Facilities are
available to investigate and manage all kind of Paediatric endocrinal problems.
There are special clinic for growth, diabetes and obesity management.

GASTROENTEROLOGY
AND HEPATOLOGY
It is a well-developed
facility in the Department of Paediatrics of our Hospital.
Services provided are
The department has
performed 10 liver transplants in past 4 years with success rate is 80%. Our
unit is credited with the first cadaveric reduced liver graft in India, first
successful liver transplant in a fulminant liver failure in a child and the
first bloodless liver transplant in the country. We perform >350 Paediatric
GI endoscopic procedures/year. Facilities for endoscopy in newborn period are
also available. The various facilities available include UGI Endoscopy,
diagnostic and therapeutic (as in sclerotherapy or band ligation for osophageal
varices, injection for bleeding ulcers etc.) sigmoidoscopy, colonoscopy,
polypectomy for polyps, foreign body removal, stricture dilation, ERCP, pH
monitoring for Gastro oesophageal reflux (including for preterms), breath
analysis for H. pylori, capsule endoscopy and percutaneous endoscopic gastronomy
(PEG) or jejunostomy (PEJ) for enteral feeding. In fact, our centre is the first
in India to perform PEG in a neonate weighing (2.8 kgs. ) Procedures like liver
biopsy (including in neonates) and cyst/abscess aspirations are routinely
performed.
Recent advances in the management of patients with GI and liver condition are
incorporated. Latest facilities for management of hepatitis (hepatitis B and
hepatitis C), fulminant hepatic failure are available . Basic knowledge and
hands on experience to the extent possible is imparted to the post graduate
students. GI manometry and Motility machine has been recently added.

GENETICS
Genetics is the new
frontier in medicine. To bring the benefits of the Human Genome Project to
patients, the department of Genetic Medicine was established in 1997. The
department has developed into the leading genetic centre in India. Samples are
received for testing from all over India, as well as from Bangladesh, Pakistan,
Sri Lanka and UAE.
A wide variety of
facilities are available ranging from Genetic counseling, cytogenetic tests,
molecular tests, HLA, immunohistochemical and biochemical testing. Prenatal
diagnosis is also performed using the above-mentioned technologies for various
chromosomal and genetic disorders

HAEMATOLOGY
AND ONCOLOGY
Paediatric haemato-oncology
sub specialty has developed since 1990. Since then we have been getting
referrals for new patients and second opinion from all over north India. Dr
Satya Prakash Yadav has joined department of Paediatrics as a consultant since
Feb 2005. We get around 50 new patients with Paediatric malignancies (leukemia +
Solid tumors) each year and many non-malignant Paediatric hematology patients as
well. We have been doing autologous stem cell transplant for solid tumors and we
are in the process of starting allogenic bone marrow transplantation. We have
facilities for diagnosis and treatment of all kinds of childhood cancers and
blood disorders. We have daycare admission facility to give short chemotherapy
on outpatient basis and give blood /platelet transfusions and also do procedures
like Bone marrow/LP under sedation. We have separate specialty clinics for
hemophilia and thalassemia.

CARE
The Paediatric
Intensive Care Unit (PICU) is one of the finest state-of-the-art PICUs in the
state. It's an 8-bedded unit with 8 ventilators with multi-monitors on each
ventilated bed. The children who are very sick are managed in paediatric
intensive care unit. The patients in PICU are admitted through emergency
department, referred and transferred from other hospitals, referred from
department consultant’s clinics and from Paediatric wards.

diseases specialist is available to guide for the management of difficult
infections and paediatric HIV and AIDS cases. All the amenities are available to
diagnose and treat other immunodeficiency and immune disorders.

NEONATOLOGY
Neonatology services
are run by a separate department of Neonatology, which comes under Centre for
Child Health. The department of Neonatology takes care of all the sick newborns
upto first 28 days of life. The department of paediatrics provides the well baby
clinic and immunization services.

NEUROLOGY
Specialised services
of child neurology and epilepsy at the Centre for Child Health have been in
existence since the past 10 years. SGRH is fully equipped with neuro radiology
services including CT, MRI and Angiography. Neurophysiology lab services include
EEG, nerve conduction and EMG facilities and evoked potentials.
We receive referrals
from in state and out state hospitals for both indoor and outdoor management of
complex cases. A Paediatric neurology OPD is run on a regular basis and has the
largest attendance amongst the specialties. The department has gained prominence
because of our expertise in behavioral neurology and epilepsy; with particular
reference to Autistic spectrum Disorders and ADHD. We have used Botox injections
in over 75 children with cerebral palsy. The availability of an excellent
genetics department has helped us to diagnose a variety of chromosomal, genetic
and neurometabolic disorders. An important aspect of this has been an exposure
of residents to these disorders.
Regular teaching of
DNB students is done with special emphasis on history taking, clinical
examination and pragmatic approach to neurological disorders. One DNB candidate
had a neurology thesis in 2003.

OFFICE
PAEDIATRICS
The office paediatrics
has been recognized has separate specialty of paediatrics by the department of
paediatrics. This division will focus on research and training into ambulatory
Paediatrics. The areas of interest include parenting, vaccinations,
growth-development, common childhood problems and illnesses faced by
paediatricians in office practice.

PULMONOLOGY
The Pulmonary Division
strives to provide a stimulating and well-rounded educational experience for
junior and senior medical students as well as paediatric residents. Division has
many years of experience in providing care to children with all forms and
severity of chronic respiratory disease. Expertise has been developed to perform
flexible bronchoscopy (diagnostic as well as therapeutic) in preterm infant to
adolescents. There is free Asthma clinic once a week.

RHEUMATOLOGY
The rheumotology
service in the Paediatric department is a specialist service to assist in
diagnosis of children with rheumatological disorders. It formulates a
comprehensive management plan with assistance of therapists, psychologists and
orthopaedic surgeon when needed. Teach/conduct bedside clinics for poor graduate
residents. They Interact with other specialist consultants including
pathologist/ radiologist/other specialist when required

NEPHROLOGY
The division of
paediatric nephrology looks after all the kidney problems of children from birth
to 16 years of age. All related emergency services are provided whether in the
ward, casualty or ICU e.g management of acuterenal failure, hypertensive crisis
or fluid and electrolyte unbalance or acidosis. All relevant procedures e.g
kidney biopsy, peritoneal dialysis (acute and chronic), haemodialysis,
plasmopheresis are done to help the patients. Peritoneal dialysis (acute and
chronic) has been done in very small low birth infants for renal failure. Kidney
transplant services are being provided with the help of adult nephrologist and
transplant surgeon. A special clinic is held every week to look after the
Paediatric nephrology patients.

THALASSAEMIA
The “Preeti Tuli
Thalassaemia Unit” of the department of Paediatrics, was inaugurated in the
year 1986. Till now, 240 patients suffering from Thalassaemia have been
enrolled, who receive comprehensive medical care, which consists of blood
transfusion, chelation therapy, regular medical check up and treatment of
complications, if any.
The Unit is equipped
with 5 beds and dedicated medical staff, who works round the clock. It runs
three shifts, morning, afternoon and evening, thus catering to 15 Thalassaemia
patients everyday. Each patient is regularly followed up and is given light
refreshment during the transfusion.
For proper and thorough medical examination, the Unit runs a “Thalassaemia
Clinic” every Wednesday, where a Hepatologist, Endocrinologist, Cardiologist,
Pulmonologist, Child Psychologist and a Paediatrician review thalassaemic
patients. Necessary investigations are done at reduced rates here. The aim of
this clinic is to monitor each patient meticulously and diagnose and treat the
complications when they arise, early, so as to decrease morbidity and mortality
and give the patients a good quality of life within the available resources.
The Unit also
participates actively in various academic activities, both nationally and
internationally. It also organizes various public awareness programmes in
schools and other public places to create public awareness about this disease.

The patient who need
short stay either for some investigation, treatment or procedure are admitted in
day care and then discharged after doing the needed intervention. There is
procedure room to conduct the paediatric procedures. Facilities for procedural
sedation and analgesia are also available in procedure room.

INDOOR SERVICES
The children needing
hospitalszation for diagnostic or therapeutic interventions are admitted to
either Paediatric wards or paediatric intensive care unit depending on
underlying disease and its severity.
Paediatric Wards
Children who don’t
need intensive care are admitted on Paediatric floor for diagnostic or
therapeutic services. There are three categories of beds in paediatric ward.
These categories include Nursing Home (NH), Semi Paying (SP) and General Ward
Beds (GW).

ADVOCACY
ON CHILD HEALTH ISSUES
The CCH has been
playing an important role on advocating following issues concerning children
Childhood
Environmental Health - We have collaborated with the WHO, Indian Academy of
Paediatrics, Ministry of Environment & Forests and the Medical Council of
India to raise the awareness of Paediatricians on childhood environmental
health, which is now a pilot project for the whole of the SEARO region.
Consumer Complaints -
Head of the Department, Dr. Subhash C. Arya, is currently the
Chairman of the Committee for Protection of Child Consumer (Indian Academy of
Paediatrics). The Department is currently involved with user-friendly marketing
and labelling of drugs.
School Health - The
department of paediatrics is actively promoting concepts of health education and
health screening at the levels of schools.
Injury and Poison
Prevention Education - The department of paediatrics actively promote prevention
strategies for injury and poison prevention in children.
RESEARCH
The past decade
witnessed a boost in research activities at Centre for Child Health. In addition
to training the DNBs in research, the CCH is now opted for many multicentric

HISTORY
OF DEPARTMENT OF PAEDIATRICS
(A DECADE OF GROWTH AND EXPANSION)
In the year 2002, the
status of department of paediatrics was raised to Centre for Child Health (CCH)
to accommodate the expanding stature and to provide comprehensive health care
from birth to 16 year. The CCH touches all the aspects related to children
during illness as well as health. CCH has expanded its concerns from therapeutic
to preventive, social and education in the field of child health. The CCH
provides best of the care to children from birth to 16 year through
co-coordinated effort of its three department, which includes department of
Paediatrics, department of neonatology and department of Paediatric surgery.
The Centre started
expanding itself 20 year backs when the process of adding Paediatric specialties
was started. Neonatology, Nephrology, Pulmonology, Paediatric Intensive Care and
Thalassemia Unit at Sir Gang Ram Hospital have already made their presence felt
15 years back. This process of expansions and the goal to achieve excellence is
continued in the present decade.
During last decade the
CCH has grown tremendously to become one the most sought after centre for
excellence in the field to postgraduate education, therapeutics, research and
social health.
Expansion of the
Faculty: The department of Paediatrics has included 10, department of
neonatology has included 2 and department of paediatric surgery has included 2
more consultants in the last 10 years.
Upgradation of
Existing Services: The Division of Neonatology has been promoted to Department
of Neonatology. Neonatal Intensive Care Unit has been upgraded with state of art
medical gadgets. The Paediatric Intensive Care Unit has been expanded to
8-bedded unit with most modern and sophisticated equipment. Thalassaemia unit
has been modernized and evening shift for transfusion services started so that
children don’t have to miss their school.
